What is IV Lidocaine?

Intravenous lidocaine is a medication used for various purposes, including pain management and treating certain heart rhythm problems. Lidocaine belongs to a class of drugs called local anesthetics, which work by blocking nerve signals in the body. Lidocaine does NOT have a “psychedelic” or dissociative effect like ketamine and can be used alone or in conjunction with ketamine and or other treatments.

IV lidocaine is FDA-approved for the treatment of certain arrhythmias (abnormal heart rhythms) and for local or regional anesthesia. Its off-label use in pain management is increasing given the number of scientific studies showing its effectiveness. Some conditions include:

  • Chronic Regional Pain Syndrome (CRPS) – We will use both Ketamine and Lidocaine together if indicated at no extra cost. Patients with vasomotor symptoms of the limb (temperature and/or color changes) tend to benefit most from added lidocaine.
  • Headache Disorders/Migraine, Chronic Daily Headache (CDH), Short-lasting unilateral neualgiform headache attacks with conjunctival injeciton and tearing (SUNCT), and short-lasting unilateral neuralgiform headache attacks with cranial autonomic symptoms (SUNA)
  • Myofascial Pain Syndrome
  • Fibromyalgia
  • Peripheral Neuropathy
  • Small Fiber Neuropathy
  • Vascular Disease
  • Post-Herpetic Neuralgia
  • Malignant (Cancer) Pain

How Does IV Lidocaine work?

IV lidocaine, when administered intravenously, works by several mechanisms to help alleviate pain:

  1. Blockade of Nerve Signals: Lidocaine is a local anesthetic that blocks sodium channels on nerve fibers. By doing so, it inhibits the conduction of pain signals along nerves, essentially dampening or blocking the transmission of pain signals to the brain.

  2. Modulation of Nerve Activity: It can alter the excitability of nerves, particularly those involved in transmitting pain signals. This modulation may reduce the sensitivity of nerves, lowering their responsiveness to pain stimuli.

  3. Anti-inflammatory Effects: Lidocaine has been observed to possess anti-inflammatory properties. In certain pain conditions, especially those with an inflammatory component, it might help reduce inflammation, thereby contributing to pain relief.

  4. Central Nervous System Effects: While the primary action of lidocaine is at the site of nerve conduction, some studies suggest that IV lidocaine might also have effects within the central nervous system, altering how pain signals are processed in the brain and spinal cord.

Intravenous lidocaine is particularly considered in cases of neuropathic pain, where nerves are affected or damaged, as well as certain chronic pain conditions where traditional pain medications may not be as effective. It’s important to note that the exact mechanisms of how lidocaine works in pain management might vary depending on the specific condition being treated.
